Midwives vs. Doulas vs. OB/GYNs: Who Does What?


Let’s set the scene: you’re planning for your baby’s arrival, and suddenly, you’re hearing terms like “doula,” “midwife,” and “OB/GYN” thrown at you left and right. It’s like assembling the ultimate birth team, but you’re left wondering: Who does what? And more importantly, who do you really need?


If you’ve ever caught yourself thinking, Is a doula like a midwife? Or do I need both?, Wait, aren’t they all the same thing?


Spoiler alert: They’re not. While all these professionals are essential in the birthing world, their roles are unique. Let’s break it down so you can feel confident building your dream birth team.


OB/GYNs: The Medical Specialists

Let’s start with OB/GYNs, the go-to healthcare providers for pregnancy and birth. They’re the ones trained to manage everything from routine prenatal care to high-risk pregnancies. If you have a medical condition or complications during pregnancy, your OB/GYN is the one who will step in to ensure both you and baby stay healthy (Cleveland Clinic, 2022).


Think of them as the captains of the medical ship - performing surgeries, prescribing medications, and managing those unpredictable emergencies that sometimes arise during birth. Midwives: Bridging the Medical and Holistic

Now, midwives are similar in that they’re healthcare providers too, but their approach is often more holistic. They focus on pregnancy, childbirth, and postpartum care, but they also emphasize natural methods and nonmedicated births (Cleveland Clinic, 2022).


Midwives are perfect for those who want a more hands-on, personal touch while still receiving medical care. Depending on your state, midwives may also perform routine health services, like Pap tests or birth control counseling.

Doulas: Emotional and Physical Support Experts

This is where we come in! Doulas are not medical professionals, and we don’t pretend to be. Instead, we focus on providing emotional and physical support before, during, and after birth. Need help with breathing techniques during labor? That’s me. Want someone to remind you how strong you are in those tough contractions? I’m your person (WebMD, 2023).


We also educate you on your options so you can make informed decisions for your pregnancy, birth, postpartum, and baby care. Whether it’s explaining the pros and cons of different birthing positions, discussing postpartum recovery strategies, or helping you understand newborn care practices, we’re here to empower you with knowledge.


Unlike midwives, we don’t catch babies, but we do advocate for you. That might mean helping you navigate your birth plan with your OB/GYN or working alongside a midwife to ensure you feel comfortable and supported.


Why the Confusion?

So, why do people often confuse doulas with midwives? Probably because we’re both known for being by your side through pregnancy and birth. The difference? Midwives handle the medical side, while doulas focus on emotional and physical comfort.


For example, during one birth I supported, the mom had a midwife handling the labor progression while I helped with counterpressure, hydration reminders, and keeping the atmosphere calm and peaceful. It’s teamwork at its best!And how do nurse practitioners fit into this? They’re advanced practice nurses who can provide general healthcare, including some pregnancy-related care. However, they usually don’t deliver babies unless they’re also trained as midwives. How We All Work Together

The magic happens when these roles collaborate. Picture this:

  • An OB/GYN oversees a complicated labor, ensuring the safety of mom and baby.

  • A midwife assists with a natural birth, offering holistic care and medical expertise.
